About the Role
Matter Health is seeking an experienced, mission-driven Market Medical Director to provide physician leadership while delivering relationship-based primary care to older adults across an assigned market.
This unique physician leadership role combines direct patient care, clinical oversight, population health, and provider mentorship. You'll partner closely with Nurse Practitioners, Registered Nurses, Patient Advocate/Liaisons, and Operations leaders to ensure patients receive exceptional, coordinated care where they live.
Our physicians care for older adults living in affordable housing communities—many of whom face significant barriers to accessing healthcare. This role is ideal for a physician who enjoys leading teams, improving clinical outcomes, mentoring providers, and helping shape the future of community-based, value-driven healthcare.
Responsibilities
Lead clinical care across an assigned geographic market
Provide physician oversight and mentorship to Nurse Practitioners and interdisciplinary care teams
Deliver primary care through a combination of in-home visits and telehealth
Lead transitions of care for high-risk patients following hospitalizations
Drive quality improvement initiatives and support value-based care outcomes
Collaborate with Operations leadership to improve patient experience and market performance
Support provider development through coaching, education, and clinical leadership
Partner with specialists, hospitals, health plans, and community organizations to coordinate patient care
Help develop clinical workflows, best practices, and care delivery strategies
Serve as a physician ambassador for Matter Health within the communities we serve
Qualifications
Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO)
Board Certified (or Board Eligible) in Family Medicine, Internal Medicine, or another Primary Care specialty
Active, unrestricted medical license (or ability to obtain licensure within the assigned market)
Minimum of 5 years of physician experience, preferably in primary care
Experience leading or mentoring clinical teams preferred
Background in value-based care, population health, or community-based healthcare preferred
Strong communication, leadership, and relationship-building skills
Passion for serving older adults and underserved populations
Comfortable working independently in both field-based and virtual care settings
Preferred Qualifications
Experience supervising or collaborating with Advanced Practice Providers
Home-based primary care or community medicine experience
Medicare Advantage or value-based care experience
Population health and quality improvement experience
Multi-site or market physician leadership experience
Passion for mentoring providers and building collaborative care teams
